Please remember to bring the following to your passport appointment:
Do not sign the form before your appointment as it must be signed in front of a Passport Agent.
Please bring a state-issued birth certificate or previous U.S. passport.
Bring a physical photo ID. (Typically a valid driver’s license)
If your photo ID is from a different state than the state in which you are applying, bring a second photo ID.
Bring two checks to the appointment!
Passport fees can be paid by check or money order payable to “U.S. Department of State”
Processing fees can be paid by check to “Probate Court” or by cash.
**For couples or families, each passport application must have a separate check or money order.
